CMS CMMI looks at new care model funding social determinants

“… HHS LOOKS AT FUNDING ‘SOCIAL DETERMINANTS’: The Center for Medicare and Medicaid Innovation is looking into a new care model that could let organizations pay for patients’ social needs, like housing and nutrition, HHS Secretary Alex Azar said Wednesday.”

“[W]hat if we went beyond connections and referrals — what if we provided solutions for the whole person, including addressing housing, nutrition and other social needs?” Azar said.
